Communication Disorders and Sciences Application Procedures
The application deadline for the CDS doctoral program is February 1. There are three parts to the CDS application process: 1) Submission of materials to the Office of Admissions 2) Submission of materials to the CDS Program, and 3) Test scores independently reported to the CDS Program
To the UO Office of Admissions, Submit the Following:
• University of Oregon Graduate School Application Form • One Set of Official Transcripts from each degree institution • $50 nonrefundable application fee
Send to: Office of Admissions 1217 University of Oregon Eugene, OR 97403-1217
To the CDS Program Office, Submit the Following:
• Department Application Form • Background Information Form • Vita or Résumé • Two Writing Samples (see Application Procedures) • Three (3) Letters of Recommendation (at least one commenting on your writing and research potential) • Three (3) CDS Letter of Recommendation Forms • One Set of Official Transcripts Send to: Academic Secretary Communication Disorders and Sciences Program 5284 University of Oregon Eugene, OR 97403-5284
Test Scores Reported Directly to CDS Program:
• GRE (Graduate Record Exam) scores. Minimum scores 500 verbal and 500 quantitative. • TOEFL (Test of English as a Foreign Language) scores (if English is second language). Mimimum score 600 on paper-based test; 100 on Internet-based test. For a full explanation of the CDS policy, please see: /feature.htm?id=2156 Please schedule GRE and TOEFL tests by December.
